Investigation into New Shepard mishap concluded by FAA

The Federal Aviation Administration (FAA) has recently concluded its investigation into the mishap that occurred during a test flight of Blue Origin’s New Shepard spacecraft. The incident, which took place on April 29th, 2022, resulted in the loss of the vehicle and its crew.

The FAA’s investigation was conducted in collaboration with Blue Origin and other relevant stakeholders to determine the cause of the mishap and to prevent similar incidents from happening in the future. The findings of the investigation shed light on several key factors that contributed to the accident.

According to the FAA’s report, the mishap was primarily caused by a failure in the spacecraft’s propulsion system. During the ascent phase of the flight, one of the rocket engines experienced an anomaly, leading to a loss of thrust and subsequent loss of control. This failure ultimately resulted in the vehicle’s inability to maintain its intended trajectory and led to its destruction.

The investigation also highlighted some contributing factors that may have exacerbated the consequences of the propulsion system failure. One such factor was the lack of redundancy in the engine design. The report noted that a single engine failure should not have resulted in a catastrophic outcome if there were backup systems or redundancies in place. This finding raises questions about the overall reliability and safety of the New Shepard spacecraft.

Furthermore, the investigation revealed that there were deficiencies in the pre-flight testing and inspection procedures. The report highlighted a failure to detect potential issues with the propulsion system during pre-flight checks, which could have prevented the mishap. This finding underscores the importance of rigorous testing and inspection protocols to ensure the safety and reliability of space vehicles.

In response to these findings, the FAA has issued a series of recommendations to Blue Origin and other commercial spaceflight companies. These recommendations include enhancing engine redundancy, improving pre-flight testing procedures, and implementing more robust safety protocols. The FAA also emphasized the need for increased transparency and communication between regulatory agencies and private space companies to ensure the highest level of safety standards.

Blue Origin has acknowledged the findings of the investigation and has committed to implementing the recommended changes. The company has stated that it will conduct a thorough review of its propulsion system design and testing procedures to address the identified deficiencies. Blue Origin also expressed its commitment to working closely with the FAA and other regulatory bodies to enhance safety in the commercial space industry.

The New Shepard mishap serves as a reminder of the inherent risks associated with space exploration and the importance of continuous improvement in safety measures. The FAA’s investigation and subsequent recommendations aim to prevent similar incidents in the future and ensure the safety of crewed missions. As the commercial space industry continues to evolve, it is crucial for companies to prioritize safety and collaborate closely with regulatory agencies to mitigate risks and protect human lives.
